Het inspannings ECG (afgekort X-ECG) zal typisch plaatsvinden op een loopband of op een fiets. De patiënt krijgt ECG electroden opgeplakt en zal gaan inspannen. Als de patiënt niet in staat is fysiek in te spannen a.g.v. een fysieke beperking (bijv. Artrose) kan er ook een chemische inspanning plaatsvinden. Dit gebeurd dan meestal m.b.v. Adenosine, een vaatverwijder, of Dobutamine.

In 2002 heeft de American Heart Association (AHA) een aantal Klasse I indicaties opgesteld voor het verrichten van een inspanningstest (1,2). Onderstaand zijn een aantal indicaties opgesomd:
 
    ** Om coronaire insufficientie te diagnostiseren bij patienten met pijn op de borst. Indien deze patienten in subgroepen onderverdeeld worden op basis van leeftijd en de aard van de klachten, varieert de a priori kans op coronair sclerose van 5 tot 89% (3). Daarom is van groot belang van te voren een risico inschatting te maken bij iedere patiënt. InspanningsECGs zijn goed bruikbaar bij patienten met een intermediate pretest probability. Positieve testen in deze patienten zijn waarschijnlijk echt positief omdat de vcoorspellende waarde van de test hoger ligt.
De test is minder bruikbaar bij patienten met een hoge of lage pretest probability. De test is overbodig om diagnose te stellen in deze patienten.
   
    ** Om prognose en beleid bij patienten met bewezen coroniare hartziekte te bepalen, inclusief patienten met behandelde en stabiele Angina Pectoris of een myocard infarct.
 
    ** Als screening middel voor coronaire  hartziekten in geselecteerde hoog risico asymptomatische patientengroepen.
 
    ** Om inspannings tolerantie en therapie te testen bij patienten met hartfalen die eventueel voor harttransplantatie worden opgewerkt.
 
Functionele inspanningtesten kunnen ook gebruikt wiorden om inspanningstolerantie te testen. Bijvoorbeeld ter evaluatie van arbeidsongeschiktheid, topsporters, of revalidatie.
 
    ** To evaluate selected patients with arrhythmias. The major indications are identification of the appropriate setting in patients with rate-adaptive pacemakers, evaluation of patients with congenital complete heart block considering increased physical activity, evaluation of patients with known or suspected exercise-induced arrhythmia (eg, congenital long QT syndrome), and evaluation of the efficacy of therapy in patients with exercise-induced arrhythmia (including atrial fibrillation) (show table 6). In comparison, exercise testing is not routinely recommended in the evaluation of survivors of sudden cardiac death. (See appropriate topic reviews).
 
*CONTRAINDICATIONS
Exercise testing is not without potential risks or discomfort to the patient. A national survey published in 1980 analyzed data from over 500,000 exercise tests [19]. For every 10,000 tests there were approximately 3.6 myocardial infarctions, 4.8 serious arrhythmias, and 0.5 deaths [19]. Thus, an assessment of the benefits and risks of testing must be made in each patient prior to the procedure. (See "Life-threatening complications" below).
 
The 2001 AHA statement and the 2002 ACC/AHA guidelines have identified absolute and relative cardiovascular contraindications to exercise testing (show table 7) [1,2,14]. Absolute contraindications include the following:
 
    * Acute myocardial infarction (within two days)
    * Unstable angina pectoris
    * Uncontrolled arrhythmias causing symptoms of hemodynamic compromise
    * Symptomatic severe aortic stenosis
    * Uncontrolled symptomatic heart failure
    * Active endocarditis or acute myocarditis or pericarditis
    * Acute aortic dissection
    * Acute pulmonary or systemic embolism
    * Acute noncardiac disorders that may affect exercise performance or may be aggravated by exercise
 
Many of the noncardiovascular disorders can be resolved prior to testing, although testing may not be appropriate under some of these circumstances. Common sense is usually sufficient to resolve most of these issues.
 
There are also relative contraindications that can be superseded if the benefits outweigh the risks (show table 7) [1,2]. As an example, low-level testing can be conducted under careful supervision to answer specific questions (eg, for risk stratification after myocardial infarction). There have been concerns about the safety of exercise testing in patients with an abdominal aortic aneurysm and those who have a recent percutaneous coronary intervention. However, neither is a contraindication to exercise testing. In a review of 262 patients with an aneurysm diameter >4 cm (average 5.5 cm), one patient (0.4 percent) with a 6.1 cm aneurysm had a contained rupture 12 hours after exercise [20]. Similarly, there were no acute coronary events in a report of 261 patients who underwent exercise testing soon after PCI with stent placement [21].
