Een ventrikeltachycardie geassocieerd met een lange QT tijd op het normale ECG. De QRS complexen draaien rond de basislijn (vandaar Torsade de Pointes). Hartfrequentie 250-350 slagen/min. Typisch geïnitieerd door een kort-lang-kort sequentie (de 2e slag heeft een langere QT tijd en de 3e slag valt in de T-top en initieert de Torsade). Overigens is er een polemiek in het tijdschrift Circulation over de correcte spelling Torsades de Pointes of Torsade de Pointes. De laatste spelling geniet de voorkeur (tenzij er meerdere asdraaiïngen optreden binnen 1 epidsode).[1]

Er bestaat ook een zeer zeldzame vorm van Torsade de Pointes die begint met een kort koppelingsinterval: Short coupled Torsades de Pointes.

Behandeling[2]

  • Withdrawal of any offending drugs and correction of electrolyte abnormalities (potassium repletion up to 4.5 to 5 mmol/liter).
  • Acute and long-term cardiac pacing in patients with TdP presenting with heart block and symptomatic bradycardia.[3]
  • Intravenous magnesium sulfate for patients with QT prolongation and few episodes of TdP.
  • Acute and long-term cardiac pacing for patients who have recurrent, pause-dependent TdP.
  • Beta blockers combined with cardiac pacing as acute therapy for patients with TdP and sinus bradycardia.
  • Isoproterenol as temporary treatment in patients with recurrent pause-dependent TdP who do not have congenital long QT syndrome.
